Chemical Dependency Evaluation

Chemical Dependency is often defined as usual progressive and chronic addiction to chemicals or substances. Often an individual suffers negative consequences as a result of their chemical use with employment, school, family life and may have unwanted involvement with the legal system. Often a Chemical Dependency Evaluation is warranted to diagnose the chemical dependency and recommend the appropriate level of care to treat the addictions or issues.

A Chemical Health Evaluation typically is 1.5- 2 hours long and relevant information is typically gathered using a:

  1. A clinical assessment
  2. Assessment tools like SASSI, CAGE, and MAST
  3. Collateral information from school, work, spouse, family member, etc

The completed Chemical Dependency Evaluation will include a summary of information; a diagnosis if appropriate and treatment  recommendations.  Evening appointments are also available for this service.
